What is coastal vision lifetime commitment?

We stand behind our surgical results. If a future enhancement or ”touch-up” is needed to maintain your vision results you will receive a same-technology enhancement procedure at no cost, if medically advisable.

How do I sign up for the coastal vision lifetime commitment?

Eligible candidates automatically receive the Coastal Vision Lifetime Commitment after completion of their refractive procedure. See Lifetime Commitment Eligibility Guidelines for complete details.

How do I maintain my lifetime commitment eligibility?

To maintain eligibility for your Lifetime Commitment, you must attend annual eye exams with your optometrist.

What is included with the lifetime commitment?

The surgical fee for a future enhancement or “touch-up” utilizing the same technology as your original procedure. Yearly exams and post-operative care after enhancement are not included, associated fees may be incurred.

Lifetime Eligibility Guidelines:

  • The patient’s original laser vision correction procedure must have been performed at Coastal Vision Medical Group. The patient must have followed the post-operative care regimen as prescribed by the surgeon or doctor who provided the post-surgical care. This surgeon or doctor must be a Coastal Vision Medical Group surgeon or a Coastal Vision Medical Group affiliated doctor.

  • The patient must have a comprehensive eye examination annually at Coastal Vision Medical Group. or at the Coastal Vision affiliated doctor who referred that patient for the original laser vision correction procedure and performed the post-operative care. Annual examinations performed by a different or non-affiliate doctor will not be accepted unless previously agreed upon with Coastal Vision. The laser commitment will become void if the interval between annual examinations is greater than thirteen (13) months. Fees for the annual exams are determined by the affiliated doctor and are paid by the patient.

  • Treatment of residual refractive conditions may not be performed if the patient can attain at least 20/40 uncorrected distance visual acuity, or is within +/-1.0 diopter of the originally targeted correction. There must be a minimum difference of at least +/-1.0 diopters of sphere or astigmatism from the originally targeted correction and/or the patient must achieve less than 20/40 uncorrected visual acuity before treatment of the residual refractive condition will be considered. The patient must attain a correction of 20/40 visual acuity or better with refraction and must have been able to obtain a visual acuity of 20/40 or better with a correction prior to treatment. No treatment of residual refractive conditions will be performed until at least three (3) months after the original treatment to assure that the post-operative refractive condition is stable.

  • Changes in vision due to pathology (cataract, auto-immune disease, keratoconus, collagen vascular disease, retinal detachment, diabetes, etc.) or injury are not covered. This agreement does not cover normal aging changes of the eye that result in a need for reading glasses (presbyopia), nor does it cover patients who have undergone Radial Keratotomy (RK), Astigmatic Keratotomy (AK), or Automated Lamellar Keratoplasty (ALK). Treatment of residual refractive conditions will not be performed if the surgeon feels it would be harmful to the patient’s health or vision.

  • Post-operative care for the residual treatment will be provided or paid by Coastal Vision Medical Group only if the retreatment is within twelve (12) months of the original treatment. The cost of any other post-operative care will be determined by the affiliated doctor who provides that care and will be paid by the patient.

  • A charge may be applied for retreatment if the patient wants new technology that becomes available, or if the patient decides to change from monovision to full distance, or vice versa.

  • This commitment is valid only at Coastal Vision Medical Group.
